Student Health Center

Women's Health

The SHC offers Women’s Health services. If you have concerns or issues regarding female or sexual health, email our clinic for an appointment at student_health_services@columbusstate.edu or call us at (706) 507-8620. Fillable forms are completed best on your computer with free Adobe software. If they are completed on your phone, they may not fill correctly. Fillable forms may be attached to an email and sent to student_health_services@columbusstate.edu. If you are not able to use the fillable option, please print the forms, complete them, and drop them off at the SHC prior to your appointment.

New Patient Forms

If you are a new patient to the clinic, or it has been more than a year since you were last seen, please fill out the additional forms below.

Services Offered

These services are included in the Student Health Fee, but there may be additional charges for lab tests and medications. Please note that the Student Health Center (SHC) is cashless. No cash will be handled in the clinic.

  • Annual examination, with/without PAP
  • Contraceptive counseling and education
  • Sexually transmitted infection (STI) testing, evaluation, and treatment
  • Emergency contraceptive counseling
  • Pregnancy testing
  • Gyn problem visits
  • Depo Injections

The evaluation, education, and counseling are included in the Student Health Fee. There will be charges for lab tests and medications.

Lab Fees

Annual Well Woman Exam with PAP and STI testing

Age 20 or younger
Free STI testing, no Pap
Age 21 – 29
$30 Pap and free STI testing
Age 30 or older
$65, Pap and free STI testing

Based on the new Pap smear guidelines set by the American College of Obstetricians and Gynecologists (ACOG), we still recommend a yearly physical including a breast exam, pelvic exam, and STI screening, if indicated. If you have ever had an abnormal Pap smear, consult with your Health Services provider concerning how often you will need a Pap smear.

Free HIV Testing offered periodically through the school year.
